What are the responsibilities and job description for the Physical Therapist position at Community Visiting Nurse Association?
Community Visiting Nurse Association, a Medicare certified home health care agency located in Somerville, NJ with a branch office in Phillipsburg, is currently seeking a Full or Part Time Physical Therapist for our Warren county location. Minimum 1 year physical therapist experience required with home care experience preferred. Candidates must possess strong clinical and critical thinking skills.
Responsibilities:
- Provides therapy services to older adults within their home/community environment
- Performs initial assessment including diagnostic tests and measurements such as ROM, transfers, ambulation, strength and endurance, balance and safety.
- Develops and implements appropriate individualized care plans and home exercise programs
- Ongoing monitoring and updates to care plan as appropriate
- Interdisciplinary case conferencing with other disciplines
- Documentation of observations, interventions and evaluations within the electronic medical record
Requirements
- Current New Jersey Physical therapist license is required.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Homecare experience a plus
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Strong computer skills, experience with EMR a plus
