What are the responsibilities and job description for the Outreach Specialist position at Community Research Foundation?
Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm (no on-call, evenings, or weekends)
Pay Rate: $24.90 to $26.21 per hour
Education: High School Diploma/GED required. Bachelor's degree in Social Work or related field strongly preferred but not required.
As an Outreach Specialist, you will:
- Meet the needs of members by providing comprehensive care management, indicated outreach, care coordination and health promotion, comprehensive transitional care, individual and family support services, crisis intervention, and refers to community and social support services.
- Conduct mobile outreach to individuals who are potentially eligible for Enhanced Care Management (ECM) services within required timelines and document outreach on a daily basis. Outreach and care coordination services may include visiting behavioral health and physical health programs, a person's residence, hospitals, crisis houses, long-term support facilities, detention centers, jails as appropriate, and other community or field-based locations.
- Oversee provision of ECM mobile services and implementation of Individualized Care Plan (ICP).
- Conduct outreach where the ECM member lives, seeks care, or finds most easily accessible and within Managed Care Program (MCP) guidelines.
- Connect ECM member to other physical and behavioral health and social services and supports a person may need, including housing when indicated. Monitors treatment adherence (including medication).
- Use Motivational Interviewing, Trauma-Informed care, and Harm-Reduction practices. Uses Psychosocial Rehabilitation (PSR) and recovery principles, as well as evidence-based practices that focus on goals specified in the ICP.
- Provide health promotion and self-management training to enrolled members
- First responder to intervene in crisis interventions as needed and indicated at the program or in the field in order to ensure the safety of members and staff. Informs/consults program management immediately regarding crises and/or unusual or critical incidents.
Additional Qualifications and Skills
- At least six months experience working in social services programs required
- Experience working with individuals with serious mental illness (SMI) or other chronic health conditions preferred
- Ability to navigate various EHRs (electronic health records) and use electronic health technology required
- Proficient computer skills, including MS Office
- A valid California driver's license in good standing, personal vehicle, proof of insurance and eligibility to be an approved CRF driver is required to fulfill the essential functions of this position
- Ability to pass criminal/background clearance
