Position: Patient Care Coordinator (Hybrid)

Location: Oakbrook Terrace, IL 60181 (Hybrid Remote)

Schedule: Full-Time, Monday–Friday, 8-hour shift

Pay Range: $18.00 – $20.00 per hour

About The Role

As a Patient Care Coordinator, you will serve as the vital connection between our providers, facility care teams, patients, and their families. Your attention to detail, compassionate communication, and dedication to patient care will ensure a seamless experience and high-quality support throughout the care journey. This is a hybrid role, blending in-person collaboration with the flexibility of remote work.

Key Responsibilities

• Support Nurse Case Managers with chart creation, data entry, and accurate documentation of patient demographics, medications, and diagnoses.
• Communicate daily with providers, care team members, patients, and families to relay updates, coordinate care, and provide timely follow-up.
• Conduct monthly wellness calls, offering kind and professional support while addressing patient and family needs.
• Maintain patient records in full compliance with HIPAA and organizational policies.
• Help facilitate patient access to medical providers, specialty care, and additional support services.
• Assist with administrative tasks and other duties as assigned to support care coordination efforts.

• Passionate about supporting patients and improving their care experience.
• Equipped with basic typing skills, computer literacy, and prior EMR experience.
• Familiar with Google Workspace applications (Docs, Sheets, Gmail, etc.).
  
  

Qualifications

• High school diploma or equivalent.
• 1 year of experience in care coordination or medical terminology (preferred).
• Ability to commute to Oakbrook Terrace, IL, as needed for hybrid work.
