About The Role
As a Patient Care Coordinator, you will serve as the vital connection between our providers, facility care teams, patients, and their families. Key Responsibilities
- Support Nurse Case Managers with chart creation, data entry, and accurate documentation of patient demographics, medications, and diagnoses.
- Communicate daily with providers, care team members, patients, and families to relay updates, coordinate care, and provide timely follow-up.
- Conduct monthly wellness calls, offering kind and professional support while addressing patient and family needs.
- Maintain patient records in full compliance with HIPAA and organizational policies.
- Help facilitate patient access to medical providers, specialty care, and additional support services.
- Assist with administrative tasks and other duties as assigned to support care coordination efforts.
- A clear and compassionate communicator across all channels—phone, email, and in person.
- Highly organized, detail-oriented, and skilled in time management.
- Adaptable and able to work both independently and collaboratively within a team.
- Passionate about supporting patients and improving their care experience.
- Equipped with basic typing skills, computer literacy, and prior EMR experience.
- Familiar with Google Workspace applications (Docs, Sheets, Gmail, etc.).
- High school diploma or equivalent.
- 1 year of experience in care coordination or medical terminology (preferred).
- Ability to commute to Oakbrook Terrace, IL, as needed for hybrid work.
