Job Summary- Community Missions has 3 Senior Residence Counselor positions throughout the agency. The positions are responsible for the implementation of trauma informed and person-centered individualized service plans. In addition, they supervise daily activities and maintain a safe and supportive atmosphere for the residents. Lead Residential Counselors work closely with residents, their families, and a network of service providers to ensure the residents receive the necessary support to achieve their personal goals.
Key Responsibilities-
· Conduct assessments of residents’ strengths and needs.
· Develop and implement individualized care plans that address resident needs and goals
· Monitor resident progress through regular follow-ups and adjust care plans as needed
· Advocate for residents to ensure they receive appropriate services and support.
· Maintain accurate and up-to-date case files and documentation in compliance with regulatory requirements and policies.
· Assist residents with daily living activities, such as meal preparation, medication management, and cleaning.
· Provide crisis intervention and support to residents in emergency situations
· Ensure compliance with all relevant policies, procedures, and regulations
· On-call responsibilities in rotation with a team
· Hours include evening, weekend, and holidays.
Qualifications-
· Bachelors degree preferred in psychology, social work, counseling, or a related field plus experience in the human services field. Associate Degree in psychology or related field plus 1 year experience in the human services field.
· Valid New York State Driver’s License with clean driving record
