Summary:
Provides support to the program team by assisting clients with achieving daily living skills and routine
activities/chores. Provides staff coverage to the program as identified by program schedule. Assists clients
with maintaining all outside appointments and activities by providing transportation and reporting back to
program staff clients’ accomplishments and information related to client care.
Essential Responsibilities and Target Outcomes:

Minimum Requirements:
Education:
High School Diploma or GED
Credentials andLicenses:
Must have valid driver’s license
Will become certified in MANDT, First Aid, and CPR.
Participate in trainings on an annual basis, as assigned.
Experience
- Successful experience working in Case Management or related field with a solid
- Beginner to intermediate experience with Microsoft Office products
- Experience in or willingness to learn how to use an Electronic Medical
