Description

SUMMARY: Coordinates and implements community integration opportunities to adults with intellectual/developmental disabilities, while providing support and assistance to promote choice and personal growth.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

  • Consults with Program Managers and Team Leaders to work on planning and organizing community activities, which allow the individual to utilize community resources, develop natural supports and achieve outcomes identified in the annual plan and programming needs.
  • Develops a weekly/monthly calendar of activities/community integration opportunities and communicates with necessary parties of all scheduled activities.
  • Provides guidance to individuals in Community Living Programs in the areas of activities of daily living/personal care (example-hygiene, dietary compliance), community integration (example-outings) and economic self-sufficiency skills (example-accurate handling of financial transactions).
  • Provides transportation for individuals to various activities through the use of an agency
  • Ensures adequate funds are available for staff and individuals for all activities.
  • Follow and maintain all safety procedures.
  • Complete all required documentation.

Requirements

QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Must be self-driven and flexible with scheduling and assigned work location.

EDUCATION and/or EXPERIENCE: Must have a High School Diploma or G.E.D. Experience working in a residential setting with individuals with developmental disabilities is preferred.

Language Skills

Ability to read and comprehend simple instructions, short correspondence, and memos. Ability to write routine reports and simple correspondence. Ability to interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to effectively present information in one-on-one and small group situations to residents and other employees of the organization.

Mathematical Skills

Ability to add and subtract two digit numbers and to multiply and divide with 10's and 100's. Ability to perform these operations using units of American money and weight measurement, volume and distance. Ability to compute percentages.

Reasoning Ability

Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

Certificates, Licenses, Registrations

Must have a valid driver's license and valid auto insurance.

OTHER SKILLS And ABILITIES

Must be able to develop effective and efficient communication skills through instruction and training, to adapt favorably to changing program needs and have genuine interest in the welfare of clients. Must be able to develop knowledge of behavior principles through instruction and training. Ability to learn basic computer operations for client data tracking, incident reporting and other processes deemed necessary by the supervisory staff. Ability to qualify to become authorized in administering participant medications preferred by not required.

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel objects, tools, or controls; and is regularly required to talk and hear. The employee frequently is required to stand, walk, and sit. The employee occasionally is required to bend stoop, squat, crawl, climb-height, reach above shoulder level, crouch, kneel, balance and push/pull. The employee continuously is required to lift up to 10 lbs. The employee occasionally is required to lift between 11 lbs. – 50 lbs. The job requires the employee to use the left and right hand for repetitive actions for simple grasping, firm grasping and fine manipulating. The employee must have the ability to drive the agency’s van. The employee must be able to provide assistance to individuals as needed such as maneuvering wheelchairs, or assisting clients in ambulating and transferring when necessary. Must be able to stay awake during the employee’s work shift.

Specific vision abilities required by this job include close vision, distance vision, and peripheral vision.
