Location: Administrative Office (In-Person)
Schedule: Monday – Friday, 8:30 AM – 4:30 PM
Travel: Frequent | On-Call: Some

What You’ll Do:
- Conduct assessments to identify clients' needs, strengths, and risks.
- Develop and implement person-centered care plans tailored to individual goals.
- Assist clients in applying for and maintaining state benefits.
- Facilitate transitions to long-term service models.
- Advocate for clients by coordinating with healthcare providers, and social service agencies.
- Participate in key meetings, including ISAs, court hearings, and medical appointments.
- Maintain accurate documentation and client records.
- Monitor service effectiveness and identify areas for improvement.
What We’re Looking For:
Education & Experience:
- Bachelor's degree in a related field with at least 1 year of experience, OR
- Associate degree in a related field with 2 years of experience
Skills & Qualifications:
- Strong interpersonal and communication skills
- Knowledge of person-centered care planning
- Ability to work effectively in a fast-paced environment
- Proficiency in documentation and record-keeping
- Familiarity with social service resources
