Housing Resources Coordinator

Reports to:  Housing Resources Manager

Employment Status Classification: Full-Time Salary, Exempt

Salary: $42,120.00 per year

Job Description Summary:

The Housing Resources Coordinator plays a critical role in the implementation of the Steps 2 Stability (S2S) program, a statewide initiative funded by MSHDA to reduce homelessness through integrated shelter diversion and housing navigation services. This position supports individuals and families experiencing a housing crisis by helping them avoid shelter entry or rapidly exit shelter into stable housing. As a full-time position you will be working a schedule of 40 hours per week between the hours of 8:30 am to 4:30 pm, Monday through Friday.

Essential Functions: 

  • Conduct diversion screenings and intakes to assess immediate housing alternatives and prevent shelter entry by completing required paperwork and conduct a full assessment for the applicable grant funding and resources. 
  • Provide strengths-based case management services, including mediation and problem-solving to identify safe, viable housing options following Critical Time Intervention and Housing First models using a strength-based, person-centered approach. 
  • Assist individuals with housing search and placement, including identifying units, scheduling viewings, and completing applications, and linking individuals to community resources related to their unique household needs. 
  • Build and maintain relationships with landlords and property managers to expand housing opportunities and resolve tenancy issues.
  • Execute all duties within the framework of Housing First and trauma-informed care principles.
  • Administer flexible financial assistance to support housing stability, including rent, deposits, and other eligible expenses.
  • Conduct follow-up case management services  at 30-, 60-, and 90-days post-placement to ensure housing retention and connect individuals to ongoing supports.
  • Maintain accurate and timely documentation in the Homeless Management Information System (HMIS) and other required databases.
  • Demonstrate flexible and efficient time management and ability to prioritize workload.
  • Consistently report to work on time and prepared to perform duties of position.
  • Have a strong work ethic that equally prioritizes the needs of participants and the needs of the organization.
  • Adhere to agency policies and procedures.
  • Participate in community-based activities when appropriate, representing Community Housing Network at community forums, workgroups, and events as requested. Some activities may include evening and weekend hours.
  • Continue to refine skills and stay abreast of best practices and program changes through additional training and community events. 
  • Explore long term affordable housing opportunities for program participants. Collaborate with local Continuums of Care (CoCs), Local Planning Bodies (LPBs), and community partners to ensure coordinated service delivery.
  • Represent the Housing Resources Department and Community Housing Network in the community.

Qualifications and Skills:

  • High school diploma or equivalent with lived experience; or bachelor’s degree in social work or human services related field; or human service-related experience and/or training, or equivalent combination of education and services with a minimum of 2 years of experience in housing services, case management, or homelessness prevention.
  • Proficiency in computer skills with Microsoft Windows, Microsoft Office, Outlook and the Internet.                                                                                                                            
  • Successfully complete required trainings enabling you to enter independently into the Homeless Management Information System (HMIS) utilized to record data for screening and working with individuals at Community Housing Network. 
  • Strong communication skills, both verbal and written, with experience interacting effectively with vulnerable populations (i.e., people experiencing homelessness, domestic violence, at-risk of homelessness, and those who have a mental or physical health disability).
  • Demonstrated ability to work independently and collaboratively in a fast-paced environment.
  • Working knowledge of the Oakland County Homeless Continuum of Care’s (CoC) Coordinated Entry System (CES) and other community resources and services related to housing and supports is essential.

  • Reliable transportation, possession of a valid driver’s license, and maintenance of automobile insurance coverage that meets organization coverage limit requirements. (Bodily Injury $100,000 each person and $300,000 each occurrence and Property Damage of $100,000 each occurrence.).

Travel:

Some travel required within the community to serve individuals to meet them where they are at, community partner locations, shelters, off-site meetings, trainings, or organizational events that would require the use of your personal automobile. CHN reserves the right to require a motor vehicle record check with any employee where driving is an essential function of this position.  

Work Environment and Physical Demands:

This position includes community-based work, in-office work and remote work (from home office).

The primary function of this job requires travelling to community partner locations, shelters, community spaces, program participant homes, and meeting program participants in the field.

While performing the functions of this job, the position is both sedentary and at times requires the ability to be active including standing, walking in the outside elements, in and out of vehicle bending, lifting and/or arranging files and office products and supplies and may require employee to periodically stand on a short step stool to access files.
