Location: Wisconsin Rapids/Stevens Point and surrounding areas
Pay $19.23/hour Mileage Reimbursement
Semi-Remote (Home Office Community Travel)
Part-Time or Full-time available

Your Role: Support Broker
As a Support Broker, you are more than a recruiter—you are a partner, mentor, and advocate helping individuals build the team that makes independent living possible.
You’ll work alongside members and families to ensure they have the tools, support, and confidence to lead their own care and navigate complex systems that impact their independence.
What You’ll Do
Attend referral meetings and conduct in-home visits with members and families
Help individuals recruit, screen, and hire their own caregivers or nurses
Support members through interviews, onboarding, and retaining quality staff
Mentor members on how to be successful employers, including managing schedules, communication, and staff expectations
Guide individuals through scheduling, timecards, and Electronic Visit Verification (EVV) systems
Assist members with Medicaid renewals, benefit applications, and navigating public assistance programs
Help individuals understand and navigate complex healthcare and long-term care systems
Maintain documentation and case notes while providing ongoing mentoring and support
Advocate for the individual’s voice, preferences, and independence in every step of the process
Your work ensures that people can remain in their homes, stay connected to their communities, and maintain control over their lives.
You’re a Great Fit If You Have
Healthcare recruiting experience (required)
5 years supporting individuals with disabilities or the frail elderly
Strong organization and communication skills
A passion for person-centered support and advocacy
Experience with self-directed or long-term care programs (a plus)
A valid driver’s license and reliable transportation
