Community Horizons is teaming up with multiple individuals in the Shawano community to find caregivers. We are seeking caregivers with all ranges of experience and availability to provide individualized care to community members. Every member we support takes part in Self-Directed Support; a program that enables individuals to build and employ their own in-home care team.
Duties include, but are not limited to:
· Assistance with bathing
· Assistance with toileting
· Assistance with hygiene
· Assistance with mobility and transfers
· Light housekeeping
· Meal preparation
