JOB REQUIREMENTS: Seeking Caregiver for Evening Hours NO WEEKENDS Do you
have a few evening hours a week to assist a wonderful man in the Green
Bay area in his well-set up home? No weekends! This position is perfect
for an experienced caregiver looking to fill their schedule with one
evening shift per week. What our member needs: Assist with personal
cares (bathing/toileting/dressing/dental care) Use of sit-to-stand
transfer Meal preparation as needed Assist with repositioning Light
housekeeping tasks ROM exercises Assist with braces What our member is
looking for: A minimum of 3 years of caregiving experience preferred
Reliability Strong work ethic Ability to follow directions Valid
driver\'s license Schedule: Tuesday evenings 5P-10P \*This position has
the potential to include Mondays, Wednesdays, and Thursdays 5P-10P\* Pay

individuals and families enrolled in self-directed care programs across
Wisconsin, including services through Managed Care Organizations, IRIS
(Include, Respect, I Self-Direct), and the Children\'s Long-Term Support
(CLTS) program. Self-direction allows participants to make their own
choicesselecting who provides their care and how those services are
delivered. While Community Horizons offers guidance and mentorship, we
are not the employer of record. Participants and families make all
hiring decisions, with a third-party fiscal agent handling payroll and

navigate self-directed care with connection, coordination, and
