We are seeking experienced Registered Nurses to provide high-quality home care services throughout Ulster and surrounding counties.

Responsibilities

• Conduct admission assessments and biannual reassessments for patients.
• Establish and implement individualized plans of care based on nursing assessments, patient needs, and physician orders.
• Provide education and support to patients, families, and paraprofessional staff.
• Conduct as-needed nursing visits for patients requiring additional support.
• Communicate changes in patients' conditions, treatments, and medications promptly to the agency.

Nurse Requirements

• Graduate of an accredited school of nursing (ADN or BSN).
• Current New York State Registered Nurse license and registration.
• Home care experience is strongly preferred.
• Reliable vehicle required to travel to patient homes.
• Willingness to travel throughout Ulster and surrounding counties.
• Bilingual (Spanish) preferred but not required.

Compensation

• Pay-per-service rates
• Per diem rate structure will be discussed during the interview.
• Additional compensation may be available for bilingual (Spanish-speaking) candidates.
• Mileage reimbursement is available; the specific mileage plan will be discussed during the interview.

Required Availability

• Weekend availability required.
