Surrogate Family Care, a NYS Licensed Home Care Service Agency, is seeking experienced Registered Nurses (RNs) to join our growing home infusion nursing program. The Home Infusion RN provides skilled nursing and infusion services to patients in their homes and authorized community settings.
This is an excellent opportunity for an infusion nurse who enjoys autonomous clinical practice, patient education, vascular access, and building relationships with patients and caregivers.
Responsibilities
- Perform comprehensive nursing assessments and reassessments.
- Administer prescribed IV infusion therapies safely and effectively.
- Assess, access, maintain, and discontinue peripheral and central vascular access devices as applicable.
- Perform ultrasound-guided peripheral IV insertion when trained, competency validated, and authorized.
- Administer IV antibiotics, hydration, specialty/biologic therapies, and other prescribed infusion treatments.
- Operate and troubleshoot infusion pumps and other infusion equipment.
- Monitor patients for infusion reactions, adverse events, infiltration/extravasation, infection, and other changes in condition.
- Respond appropriately to emergencies and activate EMS when indicated.
- Provide patient and caregiver education regarding medications, vascular access, infection prevention, equipment, and home infusion procedures.
- Evaluate patient/caregiver competency and provide re-education as needed.
- Communicate patient status and clinical concerns with ordering providers, pharmacy, agency clinical leadership, and other members of the care team.
- Maintain timely, accurate electronic clinical documentation.
- Participate in quality improvement, competency, education, and compliance activities.
- Protect patient privacy, dignity, rights, and confidentiality.
Required Qualifications
- Active New York State RN license in good standing.
- Graduate of an approved nursing program.
- Current CPR/BLS certification.
- Minimum 2 years of clinical nursing experience, with demonstrated infusion nursing experience preferred/required based on assignment.
- Strong patient assessment, IV access, infusion, and clinical judgment skills.
- Ability to work independently in the home setting.
- Reliable transportation, valid driver's license, and required automobile insurance.
- Ability to travel throughout the assigned service area.
- Completion of agency orientation, credentialing, background screening, and competency validation.
- Must be able to assume a flexible work schedule and have the ability to travel 50-80% as needed
Preferred Qualifications
- CRNI certification
- BSN
- Home infusion or home health experience
- PICC, midline, or implanted-port experience
- Ultrasound-guided IV experience
- Experience with IVIG, biologics/monoclonal antibodies, OPAT/IV antibiotics, hydration, or other specialty infusion therapies
- Specialty pharmacy experience
