What You'll Do (Key Responsibilities)
- Service Coordination: Assist individuals in identifying needs and negotiate the array of supports required to reach their desired outcomes.
- Therapeutic Support: Provide direct behavioral intervention services, skills training, and support to a dedicated caseload.
- Care Planning: Facilitate and implement individual plans of care, ensuring all services are documented and aligned with individual goals.
- Advocacy: Connect individuals with community supports and ensure planned services are effectively implemented.
- Quality Documentation: Maintain timely, neat, accurate, and objective records of all meetings and progress notes in electronic health records.
- Education: Bachelor's or Master's degree in a Human Services field (Psychology, Sociology, Social Work, Counseling, Special Ed, etc.).
- Alternative Qualification: Registered Nurses (RN) also qualify for this position.
- Driver's License: Valid Driver's License and insurable driving record required.
- Technical Proficiency: Ability to work efficiently with computers and electronic health records.
- Clinical Acumen: Strong crisis intervention skills and the ability to evaluate individual needs effectively.
- Independence: Self-motivated with the ability to work independently while managing a caseload.
- Communication: Ability to collaborate effectively with diverse teams, families, and community systems in English.
- Organization: Excellent time management and the ability to timely record data coherently and accurately.
- Tech-Savvy: Ability to work with computers and electronic health records.
- Dynamic Settings: Positions are available in both inpatient and outpatient clinics.
- Travel: Requires local travel to provide services within the Texarkana, Atlanta, and Clarksville service areas.
