Community Healthcare of Texas is currently recruiting for a Social Worker.
POSITION SUMMARY
To provide therapeutic/emotional support, education, and resource information to patients and their families; Advocate for patients’ needs/wishes with team members, family members, caregivers, and other providers.
ESSENTIAL FUNCTIONS
- Assess the psychosocial needs of the patient and family members
- Provide therapeutic counseling and emotional support to patients and family members, and education on end-of-life issues
- Contribute to and execute the Plan of Care with the family/patient
- Record daily documentation of visits and contact in accordance with Policy
- Provides referrals to community resources
- Performs in-office/non-clinical administrative duties
- Perform other duties as assigned
POSITION QUALIFICATIONS
- Bachelor's of Social Work with 1 year of applicable experience required; Master's degree preferred
- Texas State license required
- Reliable transportation and a valid driver's license.
- Hospice or healthcare experience preferred.
- Proficient skills using a computer are required
