What are the responsibilities and job description for the Inpatient LVN position at Community Healthcare of Texas?
- Assess the patient's physical, psycho-social, and spiritual condition; contribute to the plan of care in collaboration with an RN and provide palliative nursing care.
- Provide emotional support for patients and families/caregivers while instructing in end-of-life care/issues.
- Assists RN in case management of total plan of care of patient/support persons by coordinating with team members, communicating with IDG, and supervising CNAs.
- Document nursing care and significant interactions with patient support persons and team members.
- Participate in follow-up with bereaved families with various types of contact.
- Assist the RN in the admission of a patient to Hospice House, including immediate crisis management and orientation.
- Assist RN in the discharge of patients and coordination of transfer, including instructing patients/caregivers in follow-up care.
- Assist the RN in the management of multiple patients and their support persons simultaneously in crisis with acute symptoms, including pharmacology.
POSITION QUALIFICATIONS
- Licensed Vocational Nurse licensed by the State of Texas required
- 1 year of nursing experience is required
- Hospice/home health experience preferred
- Proficient skill using a computer is required; Microsoft office suite experience required
