Job Title:  Community “Retail” Pharmacist

Department:  Pharmacy

Time Commitment: Full-time

Reports To:   Community Pharmacy Manager

FLSA Status:  Exempt

Primary location: Bellefontaine, other sites as requested

 

Position Summary

CHWP utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Community Retail Pharmacist coordinates all aspects of patient’s pharmaceutical needs and provides safe, efficient, patient-centered, evidence-based medication services.

Primary Accountabilities

  • Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.
  • Ensure accuracy, efficiency, and appropriate confidentiality with patient charting and related recordkeeping and administrative functions as assigned.

Operational Excellence

  • Understand and ensure all practices and procedures related to the collection and delivery of services are consistent with contracted protocols and procedures, and that the team-based care model is followed with efficiency

Relationship Management

  • Develop and ensure favorable relationships with all patients and their families.  Achieve commitments from patients to adhere to an effective plan of care and ensure all patient services are delivered in a caring and professional manner. 
  • Develop and ensure ongoing, positive relationships and collaboration members of the team assigned.  Ensure the success team-based care and that relationships with all clinic operations staff are highly respected.

Professionalism & Stewardship

  • Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.
  • Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.

Primary Tasks and Duties                  

Pharmacy Services

  • Accurately process, fill and dispense medication pursuant to a prescriber’s order according to the laws and regulations defined by the Ohio State Board of Pharmacy and institutional policies.
  • Enter or review entry of medication orders into the pharmacy computer system and ensures accuracy of dispensing.
  • Comply with OBRA 90, HIPAA requirements and accreditation standards applicable to the pharmacy.
  • Supervise pharmacy technicians and provide answers to drug information questions for patients, families, and other health care practitioners.
  • Responsible for all necessary record keeping, maintaining of appropriate business records, supervising of all staff, maintaining all policies and procedures, inventory control, and accurate prescription filling.
  • Maintain established procedures concerning quality assurance and security of controlled substances.
  • Responsible for operations related to medication delivery or distribution to patients and departments within the organization
  • Responsible for planning, implementation and evaluation of education programs for chronic diseases.
  • Completes required continuing education pertinent to programs, certification, licensure, etc.

Clinic

Consult with patients/families/providers regarding drug therapy management or monitoring.  The clinic visit with patients includes the following components:

  • Obtain necessary patient history in order to provide safe and effective medication management
  • May do blood pressure and other vitals as deemed necessary
  • Analyze patient information and laboratory data for all aspects of medical therapy
  • Analyze factors that interfere with safe/effective medication use
  • Formulate plan of care obtaining additional lab studies, adjust medication dosages, arrange follow-up visits, provide appropriate patient education and provide intervention or referral for newly identified problems within current scope of practice
  • Discuss treatment plans with other health care providers, as necessary
  • Provide education for patients and families about medication use
  • Provide self-management education and training in chronic disease states
  • Provide appropriate phone follow-up, when necessary, on clinical issues
  • Fully document all clinical services in patient chart

Clinic Administrative

  • Supervise all pharmacy students that have a clinical rotation in the clinic
  • Work with clinic management to assure appropriate reimbursement for clinical services
  • Evaluate current clinical services through QA/I
  • Investigate and implement additional services
  • Keep Scope of Practice up to date with current practice
  • Work with clinic nurse manager on medication stock and nursing/pharmacy issue

Patient and Provider Medication Education

  • Provide education for patients, families, and providers regarding disease state pathophysiology and treatment, medication risks, benefits, and precautions. 
  • Provide medication information to medical staff as needed

Pharmacy Clinical Faculty

  • Serve as preceptor to Pharm.D. Students.  Supervise student activities and provide on-site education for students.  Support student professional and patient projects.

Other duties

  • Prepare formal educational materials and documents as they relate to job aspects listed herein
  • Review medical texts and literature as it relates to job aspects listed herein
  • As assigned

  • Engage in certification projects, i.e. PCMH, Joint Commission, HRSA
  • Engage in all corporate compliance requirements of Community Health & Wellness Partners
Qualifications:

Qualifications and Requirements

Education

  • Graduate from an accredited school of pharmacy

     Certificates and Licenses              

  • Valid Ohio driver's license
  • Registered Pharmacist in the State of Ohio
  • Experience in a community health center setting preferred
  • Three years of community pharmacy experience preferred

  • Demonstrated expertise in relevant medical practices, protocol, trends and best practices in clinical areas assigned.
  • Demonstrated knowledge and success in effecting overall clinical operations.
  • Experience/Knowledge of accreditation processes and requirements, as well as all federal, state and local regulations and standards associated with the delivery of care in a community health center environment.     
  • Ability to manage multiple responsibilities and emergency situations successfully 
  • Proficiency in knowledge of office management, use of computer, software packages, and office machines

    Physical/Environmental

  • Occasional lifting of 40 – 50 lbs. and pushing of 5-20 lbs.
  • Sitting, standing and walking approximately 80% of the time
  • A medium to high level of manual dexterity required
  • Bending and reaching approximately 20% of the time
  • Normal accessibility and mobility throughout the region required
  • Normal overtime/extended work hours

Supervision of Pharmacy Technicians at Community Health & Wellness Partners.

Work Environment

The work environment characteristics are those for a normal provider office environment. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Work may require occasional weekend and/or evening hours.

Work hours

Full-time, exempt salary position.  Generally, a 40-hour work week is required for full-time; part-time as agreed upon by parties; hours are subject to change.  Office hours vary by site.  Some Saturdays and after business hours work may be required.  Assigned meetings and travel are a part of the position.

Wages

Starting wages for Position title will be negotiable based on Community Health & Wellness Partners pay policies and factors such as education and experience, national and state average compensation recommendations based on location and region.
