Position Summary:
This position provides a range of psychosocial assessment, supporting the other behavioral health professional team members (e.g. Psychiatrist; Clinical Psychologist; Psychiatric Mental Health Nurse Practitioner; Behavioral Health Nurse), therapy/counseling services for individuals, couples, groups and/or families based upon referrals from other behavioral health or primary care providers
Essential Functions / Duties:
- Provides psychosocial assessment so patients and families to identify emotional, social and environmental strengths and problems related to their diagnosis, illness, treatment and/or life situation for those referred by staff behavioral health/primary care providers.
- Formulates, develops and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work and/or counseling treatments and interventions. Interventions may include crisis intervention, brief and long-term individual, marital, and family and/or group therapies.
- Provides therapeutic crisis intervention and emergency behavioral health services as required.
- Participates in period staff conferences to present case histories; confer with internal external members of therapy team to develop comprehensive evaluations or plan pertinent therapy programs to provide maximum benefits for each patient.
- Prepares and maintain a case record for each patient to describe the nature of social problems and services suggested or provided.
- Provides education to patients and families around issues related to adaptation to the patient’s diagnosis, illness, treatment and/or life situations.
- Maintains working knowledge of and liaison with community agencies and resources.
- Provides consultative services to health care team members within scope of care definitions needed.
- Document, assess and plan interactive interventions and treatment according to CHC/SEIA’s guidelines and standards.
- Maintains confidentiality around patient and family information.
- Maintains records and statistics in accordance as required by legal agencies and CHC/SEIA.
- Assist with the implementation of sustaining of patient centered medical home (PCMH) status as needed.
- Other duties as assigned.
Education:
- Graduation from accredited master’s social work, counseling, marriage/family therapy, and/or behavioral health education program.
Experience:
- Minimum of three (3) years’ experience with integrated behavioral healthcare environment preferred.
Licensure:
- Licensed as behavioral health professional to practice as LISW in Iowa (LISW; LMSW; LMFT or LMHC)
- BLS, ACLS or ATLS certification required.
Salary : $54,816 - $75,341
