role combines administrative support (scheduling, care coordination,

Tasks: Contact patients to schedule home visits and follow-ups. Maintain
a system for tracking visits and outreach efforts. Assist the provider
with scheduling and care planning. Update patient information in care
coordination tools and medical records. Coordinate lab draws, medical
supply needs, and other home-based services. Community Outreach and
Support (Field Work): Conduct home visits to offer support, check in,
and build relationships. Identify and assess needs related to food,
housing, transportation, or isolation. Deliver items like scales,
educational handouts, or other small equipment. Offer social connection
sometimes just a conversation makes all the difference. Report relevant
updates and concerns to the provider in a timely manner. Qualifications
Who You Are: Experienced in community outreach, healthcare, social
services, or caregiving. Friendly, dependable, and comfortable talking
with people from diverse backgrounds. Organized and detail-oriented with
strong communication skills. Familiar with Cape Cod communities and
comfortable with local travel. Licensed to drive in Massachusetts and
Have Reliable Transportation. Preferred Additional Qualifications a
Plus! Background in working with older adults or medically complex
populations. Knowledge of community-based social services and support
programs. Basic comfort with tools like Excel, Google Sheets, or EPIC
