Community Outreach and Support (Field Work):
· Conduct home visits to offer support, check in, and build relationships.
· Identify and assess needs related to food, housing, transportation, or isolation.
· Deliver items like scales, educational handouts, or other small equipment.
· Offer social connection—sometimes just a conversation makes all the difference.
· Report relevant updates and concerns to the provider in a timely manner.

· Licensed to drive in Massachusetts and have reliable transportation.
Preferred Additional Qualifications a Plus!
· Background in working with older adults or medically complex populations.
