Wage range for this position is $16.50-$23.50/hr.
Community Health Worker Duties Include:
- Providing community outreach and education to coordinate healthcare and community resources for targeted populations.
- Initiate management of Resource Requests within defined time frames set by CHC
- Ensure services are provided in targeted communities
- Coordinate health care and community resources for clients
- Assist patients with establishing CHC as a Primary Care Provider
- Utilize interpretation for any interviews of non-English speaking clients
- Educate and assist with enrollment in the Medicaid programs
Requirements for a Community Health Worker:
- Excellent computer and communication skills. Attention to detail and the ability to manage multiple projects/tasks and work independently.
- Experience working with insurances (Medicaid, Medicare & Private Insurances)
- Community involvement (past work or volunteer experience) and knows the resources in the community.
