Job Title: Case Manager II

Program:  Flexible Housing Subsidy Pool for the Ending Transgender Homelessness Initiative 

Reports to: Deputy Director of Subsidy Services

Status:  Union, Non-exempt

Hourly Wage: $29.70

Schedule: 9am-5pm, Monday-Friday

Whether one needs housing, medical shelter, a safe place to recover, or calm in the face of domestic violence - we care for those most vulnerable in our community.

 

We are not just one program. We are a network of programs designed to work together to meet every stage of need. We focus on the hardest-to-reach, most vulnerable people: Women , Medical Crisis, Substance Use Disorder, and Older Adults. Our programs meet the needs of people, rather than demanding that people meet the needs of our programs.

 

JOB SUMMARY:

Community Forward SF is hiring a Case Manager II for our Ending Trans Homelessness program. The program works with participants who are part of the trans and gender nonconforming community by providing case management, housing location and coordination services, and an ongoing housing subsidy. 

 

Case Managers assist participants with all needs related to obtaining and maintaining their housing and progress on their individual goals.  Case Managers provide services to support participants with their mental and physical health, referrals to community resources, benefits connection and support, needs assessments,  Individualized Service Plans, and other wrap-around services as needed. 

 

ESSENTIAL JOB FUNCTIONS:

  • Establish and maintain a caseload of participants, providing services including, but not limited to, advocacy, connection to community resources, benefits support, and crisis intervention. 
  • Meet with participants regularly to assist them in developing and executing an Individualized Service Plan that includes timelines for individual goals and objectives.
  • Ensure access to resources that help stabilize participants' income through benefits advocacy, housing, medical status, and other needs, including social service systems and other agencies.
  • Conduct outreach to encourage program usage, especially regarding housing placement through the program’s Housing Specialists.
  • Identify participant behaviors that may jeopardize their housing and intervene when necessary to promote housing retention. 
  • Advocate and link participants to eviction-prevention resources to help with housing retention. 
  • Maintain participant files and associated documentation.
  • Assist participants in developing external support systems.
  • Adhere to established policies and procedures.
  • Ensure that participants are treated with dignity and respect and that their needs are met while participating in the program.  
  • Support our mission to promote stabilization, hope, and healing for adults experiencing homelessness in San Francisco. 
  • Training and meeting attendance.
  • Other duties as assigned.
 

BACKGROUND & EXPERIENCE: 

  • Bachelor’s (B.A.) degree or equivalent or 4 years of housing-related experience. 
  • One year minimum case management experience. 
  • Comfort with technology including basic knowledge of Microsoft Office Suite, Google Suite, and city housing databases.
  • Knowledge of and familiarity with the ONE System
  • Effective verbal and written communication skills 
  • Ability to work both independently and cooperatively
  • Ability to de-escalate conflict 
  • Extensive understanding of homelessness, trauma, DV/IPV, HIV/AIDS, substance use disorder and recovery, mental health disorders and neurodiversity, and systems of oppression.
  • Thorough understanding of the Transgender and Gender Non-Conforming community and the specific issues and barriers they face.
  • Ability to engage with others in a positive, energetic, helpful, and trauma-informed manner.
  • Able to work effectively with diverse individuals who self-identify by culture, language, neighborhood, sexual identity, gender, and other definitions.
  • Ability to write and speak English effectively.
  • Proof of COVID-19 vaccination and TB clearance required within 7 days of hire.
  • CPR and first aid certification are required within 90 days of hire.

  • Multicultural, multilingual
  • Lived experience in the TGNC community
  • Passion for working with at-risk client populations
  • Strong self-awareness, cultural humility, and commitment to personal praxis
  • Excellent organizational and administrative skills
  • Excellent written and oral communication skills
  • Knowledge of San Francisco community resources and neighborhoods
 

PHYSICAL REQUIREMENTS: 

  • Ability to stand and walk as needed throughout the shift for extended periods of time. 
  • Ability to lift up to 25 lbs.
  • Ability to sit and use a computer, including keyboard and visual display terminal, for extended periods of time. 
  • Finger, hand, and wrist agility.
  • Ability to complete tasks in an environment where interruptions are the norm and background noise is present.
  • Ability to move throughout buildings commonly including stairs, between buildings, and out in the community.
