Purpose:  

Case Manager on the Community Support Individual Team (CSI) will coordinate and provide comprehensive recovery and rehabilitation services to consumers participating in CSI program. Interventions include a range of therapeutic and behavioral services for individuals with both severe mental illnesses and/or substance abuse disorders. Services are generally delivered in the community. Case Manager will provide services in a culturally sensitive and appropriate manner.

Minimum Qualifications:

  1. High School Diploma with at least 5 years of case management experience.
  2. Knowledge of Recovery principles, Co-occurring Disorders Treatment concepts, and/or Evidence Based Treatment Models preferred 
  3. Excellent interpersonal, verbal, and written communication skills.
  1. Valid Illinois driver’s license is required.

  6. Knowledge of and ability to use appropriate behavior management techniques
  7. Understanding of personal and professional boundaries  
  8. Ability to plan, develop and manage multiple projects

  11. Communication skills to include interpersonal, teamwork, and collaboration skills
  12. Conflict resolution and diplomacy skills

  16. Proficient in Windows environment

 

 

Duties and Responsibilities:

 

  1. Provide case management, outreach, and aftercare services
  2. Assess clients’ functioning, strengths, needs, and diagnosis
  3. Assess for indicators of abuse
  4. Develop treatment plan collaboratively with client by defining goals, specific outcomes, and time frames for treatment
  5. Apply appropriate behavior management techniques
  6. Provide culturally appropriate, competent and individualized linkage plans in accordance with each client’s age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs and culture
  7. Determine appropriate type and level of services utilizing the Level of Care Utilization System for Psychiatric and Addiction Services (LOCUS) developed by the American Association of Community Psychiatrist
  8. Facilitate psychiatric hospitalization when indicated
  9. Work cooperatively with other members of the treatment team, offering and accepting assistance when needed. 
  10. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community
  11. Assist with outcome measures
  12. Maintain compliance at all times with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
  13. Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor 
  14. Develop close working relationships with DHS and DMH funded agencies, and other mental health service providers throughout the north and west sides of Chicago and suburbs
  15. Maintain compliance with confidentiality policies and procedures at all times
  16. Maintain positive and mutually respectful relationships with other human service providers, entitlement agencies, health care vendors, and the community 
  17. Other duties as assigned by supervisor

Salary : $45,000
