Nurse RN

Community Connections Inc. is looking for a Registered Nurse to join our Day Center team based in Plymouth, MA. In this role you will support individuals with disabilities by overseeing all health-care services provided to consumers, providing direct care and in-service training in relevant medical and health-related issues, and coordinating medical services with each consumer’s primary physician or medical clinic. You will partner with individuals and businesses throughout the community and aid Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for someone with a commitment to care, integrity and serving the community.

What You'll Do as an RN:

• Dispense and document consumer medications 
• Participate in individual case record reviews on all consumers’ with program’s team
• Conduct medication reviews regularly
• Review all pertinent consumer health concerns weekly with Clinical Team
• Monitor and record vital signs and weights every six months or as needed 
• Contact physicians, family/community residences, guardians, health care professionals and service coordinators as necessary regarding specific consumer medical issues 
• All other job-related duties as assigned  

What You'll Need as an RN:

• Fully licensed with the Massachusetts Nursing Board 
• Bachelor's Degree from a four-year college or university preferred 
• 1 year's experience in a community health setting
• 1 year’s experience in a training program for individuals with developmental disabilities 
• Valid driver's license, access to a motor vehicle, and satisfactory driving record
• Excellent verbal communication and interpersonal skills
• Demonstrated ability to work independently and as a part of a team
• Strong computer and word processing skills
