Community Connection Support Services (CCSS) is an agency that provides residential and day services to individuals with Intellectual and Developmental Disabilities. CCSS is seeking a motivated and detail oriented Day Services Coordinator to join our dynamic and mission driven team at the Haven. This position plays a key role in supporting the day services program through coordination of schedules, completion of individualized service plans, conducting person centered reviews, and ensuring the accuracy and compliance of documentation.
This role also includes providing direct support services in our Day Services (Community Engagement and Day Support) programs as needed. Position Title: Day Services Coordinator
Reports To: Program Assistant
Employment Type: Full-Time, Hourly
Location: Woodlawn, VA
Position Objective:
- To assist the Program Assistant in coordinating day services operations, including completion of Individualized Service Plans, conducting person centered reviews, reviewing and approving documentation completed by Direct Support Professionals, and scheduling services to ensure authorized hours are met.
- To ensure quality service provision and meaningful engagement for individuals receiving supports in day programs.
- To fulfill a dual role by providing direct support services in Community Engagement and/or Day Services programs as needed, particularly during the latter half of each month.
Essential Duties & Responsibilities
- Complete and update Individualized Service Plans
- Conduct and document person centered reviews and planning meetings
- Review, monitor, and approve documentation completed by Direct Support Professionals.
- Ensure documentation meets regulatory, billing, and organizational standards prior to approval
- Coordinate and manage program schedules to ensure authorized service hours are met
- Knowledge of My Life My Community services.
- Read, implement, and ensure adherence to each individual’s service plan.
- Coordinate daily schedules for individuals and staff within the day program.
- Develop and maintain program schedules that promote meaningful community engagement and skill development.
- Conduct and participate in person centered reviews and planning meetings.
- Develop, monitor, and update individualized plans in coordination with the team.
- Ensure all documentation is completed accurately, thoroughly, and within required timeframes.
- Support individuals in developing and maintaining friendships and community connections.
- Assist with behavior support strategies, document, and promote positive behavioral outcomes.
- Monitor individuals’ health and physical condition and coordinate supports as needed.
- Provide oversight of transportation and community participation activities.
- Ensure the health, safety, and well-being of all individuals receiving services.
- Provide assistance with activities of daily living, including personal care needs, as required by the individual’s service plan.
- Recognize, respond, document, and report emergency or unusual conditions.
- Maintain confidentiality in compliance with HIPAA.
- Follow all agency policies and procedures.
- Demonstrate commitment to ethical service provision.
- Report suspected abuse or neglect as a mandated reporter.
- Attend and participate in required trainings and meetings.
- All other duties as assigned by supervisor.
Qualifications:
- Minimum of a High School Diploma or GED
- Experience working with individuals with developmental disabilities preferred.
- Must demonstrate strong computer proficiency
- Must be proficient in Microsoft Word and Excel, including creating, editing, formatting, saving, and managing documents.
- Must be able to pass Criminal History and Child Protective registry checks.
- Must have a valid Driver’s license with a satisfactory DMV score
- Must obtain a passing Tuberculosis screening.
Knowledge, Skills, & Abilities:
Knowledge of:
- Principles and philosophy of developmental disability services; person-centered practices; positive behavioral supports; implementation of Individualized Service Plans; documentation and data collection systems; supervision practices.
Skills in:
- Verbal and written communication; leadership and organization; documentation review; interpersonal skills, including building trust and rapport with individuals with disabilities; observing and documenting behavior; responding to individual needs; problem solving and use of available resources.
Ability to:
- Read and understand Individualized Service Plans; coordinate schedules; ensure compliance with policies, procedures, and regulatory requirements; maintain confidentiality; demonstrate a commitment to quality, community based services; adhere to policies and procedures set forth by Community Connection Support Services; recognize and adhere to Final Rule regulation 42 CFR Part 441 regarding Home and Community Based Services.
Physical Requirements:
- Must be able to sit, stand, stoop, see, hear, and smell; use hands to grasp; use arms to push and pull; use legs to walk and run; and lift, move, or assist individuals as needed, which may include lifting up to 50 pounds independently and greater weights with assistance or adaptive equipment.
