Social Services (LSW) Qualifications:

• Bachelors Degree in Social Work/Licensed Social Worker (LSW) in Ohio
• At least one year experience in the field working directly with DD/IID individuals -preferably in residential setting
• Computer knowledge and competency
• Have a genuine interest in working with Individuals with Intellectual and Developmental Disabilities.
• Time Management Skills

Job Duties and Responsibilities:

Area: Human Rights Committee

• Serve as the chairperson of the Human Rights Committee to ensure consents and other required documentation on an ongoing basis.
• Schedule, notify members, prepare agenda (in conjunction with the Director of Programmatic Services) and present information for review to the Human Rights Committee on a quarterly basis.
• Contact and otherwise notify members of the Human Rights Committee as requested and as required for consents or other necessary informational purposes per rule, regulations, and agency policy and procedures in the event that the same is necessary between meetings.
• Schedule, notify members, prepare agenda (in conjunction with the Director of Programmatic Services) and present information for review to the Human Rights Committee as necessary in special circumstances.
• Maintain complete and accurate documentation of all Human Rights Committee proceedings per requirement.
• Provide the necessary initial and annual training of Human Rights Committee members in compliance with rule, regulations and agency policy and procedures.
• Ensure that the composition of the Human Rights Committee is maintained in compliance with rules, regulations, and agency policy and procedures. Should a vacancy occur, ensure through communication with the administrative team that a required replacement be placed on the Committee as soon as practicable.
• Ensure that any information relative to the duties and responsibilities of the members of the Human Rights Committee is provided in a timely manner.
• Present all data as required during surveys or compliance reviews.

Area: Admissions

• Serve as Chairperson on the Admissions Committee as directed by the Administrator and/or Director of Programmatic Services.
• Maintain and keep timely and updated waiting lists for future admissions or applications for admission
• Prepare any background information, including assessment and/or contact with family/guardian, as directed for any potential incoming individual. Be present at any admissions meeting/s and be prepared
• to present information necessary to the individual, family, guardian related to admission to any facility and/or location operated by the agency.
• Prepare and be responsible for presenting the “consent packet” required for Social Services in conjunction with the QIDP and Director of Programmatic Services and ensure that the same documentation as required for an incoming individual is complete and filed.
• Prepare and be responsible for ensuring that the financial documentation for any incoming individual is reviewed, completed and filed appropriately.
• Prepare and be responsible to ensure that an incoming inventory has been completed within 48 hours (preferably at the time of admission) by home staff for any individual being admitted to any location.
• Complete the initial Social Service History and Report for any individual being admitted to any location within ten (10) days of admission in preparation for the initial “My Plan” / IP meeting.
• Attend the initial and ongoing “My Plan”/IP meetings.
• Ensure that LOC (level of care) packet/information is given to QIDP to be completed and returned for submission /input and complete input for billing as directed within three (3) days of admission.

Social Service Reports

• Per admissions process, gather prior social service reports for review and obtain other necessary information for comprehensive admissions social service history/report.
• Attend each CCI individual’s “My Plan”/IP meeting as requested or as deemed helpful to the individual/family/guardian to establish a rapport and familiarity with the individual and/or his/her family/guardian.

Discharges:

• Upon decision that it is necessary to initiate discharge of any given individual, coordinate the necessary information and prepare discharge notice per the rules/regulations with oversight by the Administrator.
• Follow through with any necessary appeals, etc. for agency initiated discharge and prepare to testify should it be necessary for discharge.
• If requested, assist with the follow up to any other placement to where an individual may be going following discharge, including cooperative effort with other disciplines to insure all necessary information is provided.
• Reconcile and coordinate the transfer of funds belonging to the individual on the day of or as close to the day of discharge as possible. (note: for waiver individuals, they may have outstanding bills which must be paid first) if there is a bank account or other monies

Individual Financial Duties

• Act as oversight Director for all individual financial issue
• Responsible for overseeing the reconciliation of individual in-home monies and receipts in each location at least every 60 days.
• Responsible for overseeing the reconciliation of each individual’s bank statements to disbursements and deposits at the statements are received (monthly and quarterly).
• Act as the Jobs and Family and Services Social Security authorized representative or have designee act on behalf of the agency.
