What are the responsibilities and job description for the Care Manager position at Community Care Physicians?

Our Community Care Internal Medicine Clifton Park office is hiring a Care Manager, RN to join their team. This is a full-time temporary position with a negotiable schedule from December 2025 through Spring of 2026.

Responsibilities:

  • Work in a collaborative environment to assess, plan, implement, coordinate, monitor and evaluate options and services to meet an individual’s health needs through communication and use of available resources to promote high-quality, cost-effective outcomes.
  • Coordinate the overall interdisciplinary plan of care for a patient.
  • Act as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. 
  • Identify and engage through outreach, referral or population health initiatives those patients who can benefit from care management services
  • Collaborate with patients, patient’s family and other resources in identifying their needs and expectations to develop mutually agreed upon and realistic goals.
  • Identify patients’ immediate, short-term, long-term, and ongoing needs, and develop appropriate care management strategies to address those needs. 
  • Advocate for patients and their families to receive needed services. 
  • Develop and implement an individualized Plan of Care for each patient in his/her caseload and evaluate and revise the plan as needed. 
  • Maintain a working knowledge of community-based services and resources, and the process for referring patients for services. 
  • Work closely with internal and external resources to initiate early/appropriate discharge planning. 
  • Effectively utilize community resources and support services to minimize and prevent inappropriate hospital stays and maintain maximum achievable health status.   
  • Initiate (or refer as appropriate for) patient education based on identified learning needs of the patient and/or those providing care. 
  • Adhere to applicable local, state, and federal laws, and Community Care policies governing the patient, patient privacy and confidentiality, and act in a manner consistent with the patient’s best interest. 
  • Behave and practice ethically, adhering to the tenets of the code of ethics that underlies his/her professional credential.  
  • Advocate for the patient with respect to service delivery, benefits administration, and policy making. 
  • Integrate factors related to quality, safety, access, and cost effectiveness in assessing, monitoring, and evaluating resources for the patient’s care. 
  • Maintain familiarity with current research findings and apply them, as appropriate, in his/her practice. 
  • Collaborate with the patient’s physician(s) regarding the patient’s progress and changing needs.   
  • Maintain a high level of confidentiality and work independently.

  • Ability to engage in critical thinking. 
  • Work with all departments and sites in a collaborative manner. 
  • Maintain strictest confidentiality and discretion.  
  • Excellent communication skills. 
  • Knowledge of TQM concepts, methods and tools.   
  • Knowledge of medical records and clinical care processes. 
  • Ability to work effectively as a member of an interdisciplinary team. 
  • Excellent organizational skills and strong attention to detail. 
  • Proficiency in the use of the Microsoft Office suite, especially Excel, Outlook and Word. 
  • Familiarity with medical records and clinical care processes. 

Compensation: $65,500.00 - $85,000.00 annually

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

Qualifications:

  • Current license to practice as a Registered Professional Nurse in the State of New York
  • Three years’ experience as a registered professional nurse, preferably in a primary care setting
  • Strong professional level of knowledge and comprehensive clinical assessment skills and chronic disease management
  • Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience and judgment to plan and accomplish goals
  • Must have a clean, valid driver’s license and current auto insurance and registration.
