What are the responsibilities and job description for the Registered Nurse position at Community Care Physicians?
Our Family Practice Colonie office is looking for a part-time RN to join their team! This position will begin as part-time (24 hours per week), with an opportunity to transition to full-time (36-40 hours per week). The hours are: Monday, Wednesday, Friday 8:30am-5:00pm.
**This position is eligible for up to a $3,500 recruitment and retention bonus!!**
Responsibilities:
- Document verbal conversations with patients when appropriate.
- Document information into patient record in an accurate, legible and timely manner.
- Assess vital signs and the reason for each patient visit.
- Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.
- Prepare patient for visit as appropriate, based on office/provider requirements.

Compensation: $32.00 - $45.50 per hour

Qualifications:
- Valid NYS RN license.
- Previous experience in an RN role.
- Strong computer skills.
- Knowledge of basic nursing principles and protocols.
- Knowledge of medical chart reporting/records.
- Skill in appropriate assessment and assistance techniques.
