What are the responsibilities and job description for the Breast Center Navigator position at Community Care Physicians?
Our ImageCare Latham office is looking for a Breast Center Navigator! The hours are Monday-Friday 8:30am-5:00pm.
Summary: The Breast Health Patient Navigator’s primary function is guiding breast health patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes.
Essential Functions include, but are not limited to:
· Initiate communication with patients upon learning they have a suspicious mammogram or a positive finding.
· Guide patients through the health care system; help patients arrive at scheduled appointments on time and prepared.
· Connect patients to community and social support services.
· Facilitate interaction and communication between our breast imagers and both hospital-based and practice-based staff and providers.
· Provide breast health education to individuals and groups.
· Develop and maintain a learning library for patients that include language-specific materials.
· Identify personnel in departments involved in the care of breast health patients (i.e., physicians, nurses, radiology staff, social services staff, radiation oncology staff,
hematology/oncology clinic staff) and develop relationships with them; offer educational sessions to inform practitioners of breast health patient navigator role, services provided and encourage referrals.
· Track interventions and outcomes.
· Coordinate program planning with appropriate departments.
· Prepare documents: posters, brochures, policies, procedures, forms, etc.
· Implement support systems, referral processes, (outreach strategies), etc.
· Track responses, appointments, and other relevant data
Additional Duties:

Qualifications:
· High school diploma or equivalent.
· Successful completion of navigator course and certification as a Breast Center Navigator.
· Two years of experience in a similar breast center role.
· Two years of experience in coordinating care needs.
· Two years of experience in program development.
