Direct Support Professional (DSP)

Community Care Partners is seeking a compassionate and dependable Direct Support Professional (DSP) to provide one-on-one support to an adult participant in Port Monmouth, NJ 07758.

As a Direct Support Professional, you will provide support in the community while promoting independence, dignity, and quality of life. Services are delivered in accordance with the participant's individualized service plan.

Work Location

The Direct Support Professional will provide one-on-one services in the community. Support may include community activities, social engagement, daily routines, and other activities based on the participant's individualized service plan.

Essential Duties / Case Specifics

• Provide one-on-one support during community activities and outings.
• Accompany and assist the participant with meaningful activities in the community.
• Encourage independence, confidence, and active participation.
• Promote a safe, respectful, and supportive environment.

Responsibilities Include, But Are Not Limited To

• Providing supervision and support based on the participant's individualized needs.
• Assisting with daily living activities and routines as needed.
• Accompanying the participant during community activities.
• Encouraging independence and positive engagement.
• Promoting dignity, respect, inclusion, and personal choice.
• Maintaining professional and accurate documentation.
• Communicating important updates as required.

Physical Requirements

• Must be capable of reaching, bending, lifting, and performing light physical tasks as needed.

Experience, License, and Certification Requirements

• Must be proficient in English, including reading, writing, and comprehension.
• Experience supporting individuals with intellectual and developmental disabilities is preferred.
• Must successfully clear all required State/Federal criminal background checks, the Central Registry, and the CARI database.
• CPR/First Aid certification is preferred. Applicants may apply without certification but must obtain it before starting services.
