Case Manager/Trainer

Salary: $55,000 - $70,000 per year (Based on experience, licensure, supervisory background)

Community Care Partners is seeking bilingual English/Spanish Case Managers / DSP Supervisors for the following regional service areas:

* Union / Newark

* Matawan / Perth Amboy

* Ocean County / Forked River

These positions support individuals receiving Community Based Supports, Individual Supports, and Supported Employment services through the New Jersey Division of Developmental Disabilities (DDD).

This role is ideal for candidates with experience in social work, behavioral health, care coordination, human services, or developmental disabilities who enjoy relationship-building, mentoring staff, supporting families, and ensuring high-quality service delivery in the community.

Unlike traditional office-based case management positions, this role focuses heavily on field supervision, Direct Support Professional (DSP) mentorship, family engagement, and service quality oversight.

* Crisis Prevention and Intervention / Crisis Management Training

* Medication Administration Trainer Certification

* DSP Coaching and Field Supervision

These certifications and trainings will allow staff to provide protocol-based education, coaching, and supervision to Direct Support Professionals in the field.
