Community Care Network is looking for a Community Clinician to join our team!

About The Role

The Community Clinician provides assessment, treatment planning, and therapeutic services to adult clients in their homes or community settings, with a primary focus on older adults experiencing transportation or mobility barriers. This role helps ensure individuals have access to high-quality mental health and substance use services where they are most comfortable. Working closely with the Council on Aging and an interdisciplinary clinical team, the Community Clinician enjoys the flexibility and independence of community-based work while benefiting from a collaborative team environment, supportive supervision, and dedicated office space for documentation and care coordination.

Responsibilities

  • Provide evidence-based, individualized psychotherapy in accordance with best practices.
  • Conduct intake assessments, evaluations, diagnostic assessments, treatment planning, and referrals as clinically indicated.
  • Adhere to program philosophy and established admission, continued stay, and discharge criteria.
  • Complete clinical documentation, recordkeeping, and data collection in accordance with agency policy and the requirements of state, federal, payer, and accrediting bodies.
  • Complete documentation in a timely manner, including treatment plans, progress notes, timesheets, managed care authorizations, discharge summaries, and other required records.
  • Coordinate follow-up care, aftercare planning, referrals, and discharge services to support continuity of care.
  • Facilitate client access to community resources and additional supports.
  • Provide targeted, short-term case management services as clinically appropriate.
  • Deliver services in the client's preferred setting, primarily through home visits and community-based locations.
  • Collaborate effectively with internal departments and external community partners, including the Southwestern Vermont Council on Aging, to coordinate client care.
  • Participate in regular clinical supervision, consultation, team meetings, department meetings, and agency meetings; seek guidance and support as needed.
  • Maintain professional standards of conduct, ethics, confidentiality, and compliance with agency policies and procedures.
  • Provide education and training to staff in areas of expertise as requested.
  • Participate in community outreach and educational activities as requested by the supervisor.
  • Perform other duties as assigned.

Qualifications

  • Masters Degree in Social Work, Clinical Mental Health Counseling, Counseling, Psychology, or a closely related field required.
  • Licensed preferred; License Eligible (Rostered as a Non-Licensed Psychotherapist) in the State of Vermont required.
  • Knowledge of the assessment and treatment of mental health, substance use, and co-occurring disorders.
  • Knowledge of and sensitivity to the unique behavioral health needs of older adults and geriatric populations.
  • Ability to provide recovery-oriented, person-centered counseling and support to clients and their families in community-based settings.
  • Ability to establish therapeutic relationships with clients from diverse backgrounds while maintaining appropriate professional boundaries.
  • Strong clinical assessment, treatment planning, and crisis intervention skills.
  • Excellent written and verbal communication skills, including timely and accurate clinical documentation.
  • Strong organizational and time management skills with the ability to prioritize competing responsibilities and manage an independent caseload.
  • Ability to work both independently and collaboratively as part of an interdisciplinary team.
  • Ability to provide high-quality, client-centered services in a fast-paced environment with limited resources.
  • Proficiency with electronic health records and standard computer applications.
  • Must have a valid driver’s license, and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.
