Community Care Network is looking for an Emergency Services Clinician to join our team!

About The Role

This position is responsible for providing mental health crisis assessment, intervention and brief treatment services to a diverse population of clients in crisis. In addition, the crisis clinician will provide appropriate recommendations for care and linkage services for clients requiring on-going treatment. Emergency services are available to clients 24 hours a day, 7 days a week.

Responsibilities

  • Conduct program specific screening and evaluation and formulate diagnosis in accordance with the Diagnostic Statistical Manual (DSM).
  • Provide brief case management, service coordination and community liaison services to assure that clients are dispositioned to appropriate levels of care and according to clinical practice standards.
  • Triage clinical emergencies and implement program specific safety and consultation protocols.
  • Utilize best practices and evidenced based practice models where appropriate and adheres to model and program expectations.
  • Provide mobile service in community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program. May transport clients to appointments or linkages as appropriate.
  • Develop and document, clear clinical formulations, crisis and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant.
  • Demonstrate knowledge of internal and external levels of care, referral processes and resources.
  • Make clinically appropriate referrals for identified service needs in a timely manner.
  • May participate in community-based meetings and conference calls with emergency departments, inpatient units, congregate care providers, other providers to assist in care planning and to address system issues which may impact increased emergency department and inpatient unit utilization of high needs.
  • Communicate and collaborate in a positive, effective manner with clients, family, and all internal and external providers.
  • Attend and participate in scheduled supervision and required staff meetings.
  • Seek additional supervision or consultation as needed and follow through with supervisory directives.
  • Attend all trainings as required by the Agency, the program, or other entity.
  • Continue to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expressions of groups served as well as use appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.

Qualifications

  • Master’s degree in clinical psychology, social work or related field is preferred.
  • Experience working in crisis, emergency room, or community-based services preferred.
  • QMHP designation preferred. It is required that once hired the clinician work toward obtaining a QMHP designation.
  • Obtain Allied Health Privileges or status as employee of RRMC after hire.
  • Demonstrates knowledge of psychopathology, crisis theory and other behavioral health concerns and primary modalities of psychotherapeutic intervention.
  • Experience in crisis intervention services, risk management, assessment of suicide potential and integrated treatment for co-occurring disorders preferred.
  • Ability to join with clients and families in distress and work with them in disposition planning.
  • Must be willing to provide services in clients' homes and community settings.
  • Must be available to work shifts assigned including overnights and weekends.
  • Possess excellent communication and organizational skills.
  • Ability to function autonomously in stressful situations and make appropriate assessments in coordination with on-call physicians.
  • Must have the ability to utilize computer-supported collaborative work software and have demonstrated proficiency with Microsoft Office Applications and Electronic Medical Record software.
  • Knowledge of maintaining and executing confidential information using HIPPA standards.
  • Must have a valid driver’s license, and reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.

Competitive Pay Range - $65,000.00-$73,000.00 (compensation is typically dependent upon experience)
