RN Case Manager Job Responsibilities:
- Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessments as needed and consults w/ families and other supports for additional information as needed.
- Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
- Education: Associate’s Degree in Nursing Required, Bachelor’s Degree preferred.
- Licensure: Wisconsin Licensed Registered Nurse
- Licensure: Eligibility for long-term care functional screener certification is required
- Licensure: WI Driver’s License
- Experience: Required 2 years’ recent experience with older adults or individuals with physical or intellectual disabilities in a nursing capacity; Preferred 2 years of work in community nursing
- Hours of Operation: Monday-Friday 8:00 a.m. - 4:30 p.m.
