On-Call Triage Runner Nurse - Week Night Shift

Be apart of a reputable & compassionate local hospice agency seeking a dedicated On-Call Weeknight Triage Runner Night Nurse to provide professional and empathetic support to patients and families during overnight weekday hours. This role involves telephone-based triage, problem-solving, and in-person visits in the event of a death or PRN needed. Hours would start at 5 PM until following morning at 9 AM.

This position offers the opportunity to make a meaningful difference in the final stages of patients' lives by providing peace, care, and compassion.

Responsibilities

• Handle after-hours patient and family PRN emergency visits, assessing medical concerns and providing appropriate pain management guidance.
• Communicate effectively with caregivers to address symptoms such as pain, anxiety, or breathing issues.
• Provide emotional support and clear instructions to families during urgent situations.
• Maintain accurate documentation of all calls and actions taken.
• Coordinate with the care team for follow-ups as needed.
• Perform on-site death visits when necessary, including completing required documentation and offering emotional support to families.

Experience
The ideal candidate will possess the following skills and qualifications:

• Active nursing license in California (LVN, LPN, or RN).
• Previous hospice, palliative care experience is highly preferred.
• Exceptional communication and assessment skills.
• Ability to remain calm and compassionate during emotionally challenging situations.
• Strong problem-solving abilities and clinical judgment.
• Reliable transportation for visits.
• Strong knowledge of anatomy and physiology.
• Experience with medical documentation practices.
• Familiarity with outpatient clinic settings.
• Background in geriatrics is preferred.
• Proficiency in venipuncture techniques.
• Understanding of hospital medicine principles.
• Commitment to providing high-quality patient care.
• Ability to manage multiple tasks efficiently while maintaining attention to detail.
