We address a range of mental health challenges within home and community environments, to meet the needs of those we serve.Requirements/ResponsibilitiesProvide individual and family counseling utilizing a variety of modalities in addressing a diverse population, including complex trauma, social determinants of health, underrepresented populations and social challenges.Collaboration with internal and external providers to enhance care. Provide crisis assessments, risk management, and safety planning.
