Youth & Young Adult Coordinator, Health Transitions in the Integrated Health Clinic
Community Alliance has an exciting opportunity for a person who self-identifies as a current or former recipient of professional behavioral health services which has led toward recovery from a mental illness and/or co-occurring mental illness & substance use disorder to contribute their lived experience, perspective, knowledge, and skills to the rehabilitative and recovery process targeted toward transition age youth and young adults (ages 16 to 25).
Duties for Youth and Young Adult Coordinator, Health Transitions Program in the Integrated Health Clinic
Lead team of peer care coordinators in providing community-wide and direct one-to-one outreach to transition-aged youth and young adults to engage or re-engage them in behavioral health or related services including:
1. Utilize evidence-informed and evidence-based screening and assessment tools for the identification of at-risk transition-aged youth and young adults, including those who have been exposed to trauma, are at risk of suicide, or are at risk for substance use.
2. Assist in identifying the unique wants and needs of the person served and proactively identify and navigate community resources and other systems; make and follow-up on referrals to needed treatment services
3. Provide individualized, culturally competent support and skill building in such areas as crisis management, relapse prevention, and wellness self- management.
- Provide leadership and support for youth/young adults and in system initiatives on the agency and community level, expanding the voices and impact of population of focus, including:
1. Coordinate youth and young adult involvement on the Advisory Council and in peer-led evaluation activities.
2. Serve as part of the Healthy Transitions Care Coordination Team to facilitate identification and mobilization of recovery support services to address housing, transportation, peer and family support, vocational and education support and other resources,
Gather, record, and submit quality improvement and evaluation data, including NOMS data, in accordance with the guidelines, format, frequency and timelines required. - Possession of current, valid driver’s license, reliable transportation, and good driving record.
Qualifications for Youth and Young Adult Coordinator, Healthy Transitions Program.
- Self-identified as a current/former recipient of professional behavioral health services which has led toward recovery from a mental illness and/or co-occurring mental illness & substance use disorder.
- Successful completion of agency-recognized peer specialist training program and relevant practicum/internship; state certification as a peer specialist.
- At least one year relevant experience and two years in a position demonstrating leadership experience.
- Ability to learn and facilitate use of identified evidence-based resources and tools including Critical Time Intervention, Wellness Recovery Action Plan, and Seeking Safety.
- Familiarity with community resources.
- Ability to work well and contribute within an inter-professional team, communicate effectively both orally and in writing, in one-on-one, group, and public situations, and with persons of diverse backgrounds and perspectives.
- Excellent written and verbal communication skills.
- Ability to connect, communicate, and effectively work with people from diverse cultural, ethnic, socio-economic, educational, and professional backgrounds, and ability to maintain effective working relationships with diverse individuals and groups.
