Community Alliance, offering an integrated health care approach built on an array of mental health and primary care services, is seeking a Care Coordinator for our Integrated Health Care Services Rapid Response Team. Ideal candidates will have a passion for working with vulnerable populations and be comfortable meeting and interacting with all persons seeking services. This position will collaborate with other multi-disciplinary professionals to empower clients to achieve self-defined goals and improve their whole person health, wellness, and recovery outcomes.

Schedule: Monday - Friday, 8:30 am - 5:00 pm. No weekends and No on-call.

The Care Coordinator in Community Alliance's integrated health care services Rapid Response Team (RRT) is responsible for providing direct and collateral care coordination services which promote personal physical and behavioral (mental and substance use) health, wellness, and recovery for individuals (all ages) with mental illness and co-morbid physical health conditions. An individual in this role meets with call-in or walk-in persons interested in receiving services from Community Alliance. The RRT Care Coordinator will complete the centralized intake process and paperwork with the person and take steps necessary to get the person enrolled into the requested services/programs. The RRT Care Coordination will provide “engagement” type activities (e.g. phone calls, meetings) with the person during any waiting times until the person is able to enroll and start participating in the desired services.

  • Provide centralized intake functions with persons to quickly assess and screen for needs, complete required paperwork, document information in the electronic health record, and request services/programs for people to receive services. Additionally, following up with the person as needed to support and encourage them until s/he is able to enroll and start in services.
  • Assist persons served in identifying, accessing, and utilizing primary, behavioral health, specialty health services, and other services and supports related to the social determinants of health (food, housing, education, employment, social networks and supports, etc.)
  • Actively assist persons served in effective communication and self-advocacy with primary and behavioral health providers. Advocate for the person and assist in navigating to services.
  • Assist individuals in making and remembering appointments, arranging transportation, and keeping scheduled appointments for services.
  • Provide individualized, strength-based support, education, coaching, information, and advocacy services.
  • Continuously develop and utilize knowledge, skills, and competencies necessary to perform position responsibilities in a high quality fashion and consistent with organizational, accreditation, and professional standards.
  • Bachelor’s Degree in Social Work, Rehabilitation, Psychology, Education, or other equivalent area of study from an accredited academic institution, OR equivalent years of experience in roles and responsibilities congruent with the requirements of this position.
  • Demonstrated knowledge of characteristics and needs of persons with mental illness.
  • Demonstrated knowledge and skills in mental health rehabilitation and recovery principles.
  • Ability to transport self to and from community sites to perform position responsibilities including possession of valid driving license, good driving record, and reliable transportation, or the ability to arrange for such transportation on a regular basis.
  • Ability to maintain effective working relationships with individuals of diverse backgrounds including persons with mental illness, behavioral health and physical healthcare professionals, family members, social service providers, and public at large.
