Commonwealth Home Health Network is looking for Occupational Therapists to join our team. CHHN provides in-home, skilled and unskilled services to seniors, older adults, and those who are disabled.
- Provide care for our community of recovering patients, ushering them back to a life of independence.
- Create and follow personalized plan of care in collaboration with RN and the interdisciplinary team.
- Educate our patients and their families on next steps in continuing care and improvement of day to day life including: therapy, exercise, and treatments.
- Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.
- Focus on the goal of improving the overall health of our community.
- Oversee Occupational Therapy Assistants.
- Perform all duties with a passion for compassion.
- Valid Occupational Therapy license (required)
- 1 years of OT experience
- A heart for providing the best care to our community
