What are the responsibilities and job description for the Targeted Case Manager position at CommonHealth Recovery?
Targeted Case Manager (Care Navigator)
CommonHealth Recovery | Frankfort, KY | Full-Time | Monday–Thursday | $18–$24/hour
Who We Are
CommonHealth Recovery is a Kentucky-owned outpatient behavioral health and addiction treatment organization with offices in Frankfort and Lexington. We started in 2020 because we believe people in recovery deserve care that meets them where they are. We are licensed by the state as an AODE and DUI treatment provider, and we serve adults covered by Medicaid, Medicare, and commercial insurance.
Our services include medication-assisted treatment (MAT), intensive outpatient programming (IOP), individual and group counseling, addiction psychiatry, DUI education and treatment, and targeted case management. Our care is evidence-based, trauma-informed, and grounded in harm reduction. We focus on progress, not perfection.
Why This Role Matters
Treatment works best when a person's basic needs are met. Someone without a ride can't get to group. Someone facing eviction can't focus on recovery. Someone whose Medicaid lapses loses access to their medication. As a Targeted Case Manager, you remove those barriers. You connect our clients to housing, transportation, benefits, employment, and medical care so they can stay in treatment and build a stable life.
You'll work on a multidisciplinary team with counselors, nurse practitioners, a collaborating physician, and front office staff. Your work directly affects whether clients stay engaged, and we treat it that way.
What You'll Do
Client Assessment and Care Planning
- Complete Medical Necessity Assessments (MNAs) and ongoing evaluations of housing, transportation, food security, employment, legal, medical, and other social determinants of health.
- Develop, review, and update individualized TCM treatment plans at least every 30 days, matched to each client's goals and clinical recommendations.
- Screen for chronic and complex health needs and build safety and crisis plans when needed.
Resource Coordination and Advocacy
- Connect clients with community agencies, housing programs, primary care, employment services, and recovery supports, and follow up to make sure the connection holds.
- Help clients apply for and keep benefits, including Medicaid renewals, SNAP, housing applications, ID and document recovery, and transportation programs.
- Advocate for clients with outside agencies, courts, probation and parole, and community partners, with the client's authorization.
Treatment Team Collaboration
- Coordinate closely with counselors and medical providers so care is aligned across MAT, IOP, and outpatient programs.
- Take part in treatment team meetings, case consultations, and clinical supervision.
- Support client retention through engagement outreach, appointment reminders, and re-engagement follow-up for clients at risk of dropping out.
Crisis Support
- Provide crisis intervention and de-escalation, and help clients identify and work through barriers to staying in treatment.
Documentation and Compliance
- Document all services in our electronic health record (AdvancedMD) the same day whenever possible, and no later than 48 hours after the service.
- Keep documentation that meets Kentucky Medicaid, managed care, and payer standards, including support for prior authorization requests.
- Protect client privacy under HIPAA and 42 CFR Part 2.
- Other duties as assigned.
What You Bring
Required
- Bachelor's degree in social work, psychology, counseling, sociology, human services, or a related behavioral science field.
- Ability to complete Kentucky's state-approved Targeted Case Management training within the required timeframe after hire. We pay for it.
- Strong written communication and the discipline to keep documentation timely, accurate, and organized.
- Reliability, punctuality, and follow-through. Our clients depend on it.
- Ability to pass a background check and drug screen.

- At least one year of experience with people who have substance use disorders, serious mental illness, or other high-risk needs.
- Knowledge of Kentucky community resources, Medicaid, and benefits systems, especially in Franklin County and Central Kentucky.
- Experience with an EHR. AdvancedMD experience is a plus.
- Lived experience of recovery is valued here.
Who Thrives Here
- You care about people and stay steady when things get chaotic.
- You treat every client with dignity, cultural humility, and no judgment.
- You like structure, but you can adjust when a client's day falls apart.
- You want to grow your career and are open to feedback.
