We're looking for an organized, compassionate, and customer-focused Homecare Services Coordinator to join our team. What You'll Do
As a Homecare Services Coordinator, you'll serve as the central point of communication between our clients, families, caregivers, and office team to ensure exceptional care is delivered every day.
Your responsibilities will include:
Client Care Coordination
- Coordinate and oversee daily in-home care services for assigned clients.
- Maintain an understanding of each client's care needs, preferences, and service history.
- Monitor quality and continuity of care, proactively resolving service issues.
- Communicate regularly with clients and families to ensure satisfaction and respond to changing care needs.
- Support new client service implementation and ongoing care coordination.
Scheduling & Caregiver Support
- Match caregivers with clients based on skills, availability, location, and client preferences.
- Manage daily schedules and respond quickly to call-offs, schedule changes, and coverage needs.
- Support caregivers by answering scheduling questions and providing timely communication.
- Help maintain consistent caregiver-client relationships whenever possible.
Administrative & Compliance
- Maintain accurate schedules, service documentation, and client records.
- Ensure compliance with agency policies, HIPAA requirements, and state home care regulations.
- Assist with client onboarding and coordination with supervisory staff.
- Support quality assurance activities and routine record audits.
Communication & Teamwork
- Serve as a trusted resource for clients, families, caregivers, and leadership.
- Communicate important updates regarding client needs, caregiver availability, and service changes.
- Collaborate with agency leadership to continuously improve service quality and caregiver engagement.
Emergency Response
- Respond to urgent client care or staffing situations as needed.
- Participate in after-hours on-call scheduling support on a rotating basis.
What We're Looking ForRequired Qualifications
- High school diploma or GED.
- Minimum of two years of experience in home care, healthcare coordination, or a related administrative role.
- Strong organizational and time management skills.
- Excellent communication and customer service skills.
- Ability to prioritize multiple tasks in a fast-paced environment.
- Proficiency with Microsoft Office and scheduling software.
- Ability to work independently while collaborating effectively with a team.
- Flexibility to assist with urgent scheduling needs before or after regular business hours when necessary.
Preferred Qualifications
- Post-secondary education in Healthcare Administration, Social Work, Nursing, or a related field.
- Experience working with seniors or individuals with disabilities.
- Familiarity with home care regulations and industry best practices.
