Job Summary:
The Client Care Coordinator is responsible for customer service and relationship management ensuring that clients receive the highest quality of care, and satisfaction with care. The CC Coordinator is the initial and ongoing contact, via phone, email, or in person, with the client, CM, family, or referral source. The position is responsible for completing client intake in the home, monitoring care quarterly, and communicating care coordination details to all team members, case managers, and family members.

Essential Functions:
A successful Client Care Coordinator
- Assists with beginning the new client intake process. Coordinates and completes client intakes and annual paperwork from start to finish, and anticipates needs and provides support to clients and their families.
- In cooperation with agency team members, and Case Manager of the client, the CC Coordinator will complete, & review CDAC agreements for accuracy.
- Gathers, reviews and submits 90-day documentation to case managers when requested.
- Identifies initial safety concerns and makes recommendations regarding the client and their home.
- Correspondence with Families, Guardians, Case Managers, Discharge Planners, and any other Referral source.
- Maintain Client Home Visit Schedule and visit clients annually or as needed.
- Completion of a percentage of Quality Assurance Reports, including the review of quarterly case notes. Will communicate to case managers when we need client paperwork that is missing for our company files.
- Markets the PERS program to new client prospects as well as existing clients.
- Assists with quarterly calls and client case noting (concerns, ongoing issues, hospital stays, incidents, etc.).
- Enters intakes and tracks prospects via spreadsheet/tele track. Communicates client home assessment and other needs to management team.
- Follows all office communication and documentation protocols to ensure excellent service to potential, current, and inactive clients.
- Participates in formal and impromptu management and caregiver meetings to assist the development of systems to improve care coordination, client satisfaction, employee performance and retention and office operations.
- Answers potential client inquiries/referrals whether made via phone or email, and sells and qualifies them for In-Home Care Visits.
- Assists General Manager with the collection and review of SCL logs/documentation.
- Assist with the transition/discharge planning
