POSITION SUMMARY: The Community Health Representative (CHR) Director is responsible for the administrative and operational oversight of the CHR program, including supervision of staff, budget management, regulatory compliance, and program development. This position is primarily administrative in nature, with an emphasis on leadership, coordination, and organizational management.

The Director oversees the planning, implementation, and evaluation of community-based services while ensuring compliance with tribal, federal, and organizational policies. The role requires ongoing collaboration with tribal leadership, Indian Health Service (IHS), and internal departments, and involves navigating complex organizational structures.

In addition to supervising CHR staff and supporting public health initiatives, the Director manages program resources, reporting requirements, and operational workflows. While the program supports direct patient services, this role functions largely in a supervisory and administrative capacity with occasional hands-on care.

Community Health Representatives serve as a bridge between patients and access to healthcare services and essential resources, while the Director ensures the systems, staffing, and partnerships are in place to support effective service delivery and improved community health outcomes.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Provide leadership, supervision, and support to CHR staff
  • Coordinate care and referrals between patients, healthcare providers, PRC, and other services
  • Ensure compliance with tribal, federal, and organizational policies and guidelines
  • Collect, analyze, and report data to support program evaluation and improvement
  • Attend CBC meetings (when necessary) to speak on department’s behalf
  • Develop and maintain Standard Operating Procedures (SOPs) for program operations
  • Collaborate with internal departments, tribal leadership, and external partners (e.g., IHS, public health agencies)
  • Refer patients to internal and external departments that will benefit them
  • Manage program budgets, resources, and grant requirements
  • Advocate for community health needs and culturally competent care
  • Develop and implement community health programs and outreach initiatives
  • Ensure accurate and timely documentation of services provided
  • Oversee government vehicle fleet and maintain communication with GSA representative
  • Schedules patient appointments efficiently while prioritizing acuity, availability, and continuity of care
  • Answers and triages incoming phone calls, messages, and inquiries in a professional and timely manner.
  • Provides direct patient support, including checking on patients, responding to concerns, and ensuring comfort and safety.
  • Transports patients, medical supplies, and medications in accordance with safety and facility protocols.
  • Ensures timely delivery and availability of medical supplies and medications necessary for patient care.
  • Acts as a point of escalation for staff questions, workflow issues, and patient concerns.
  • Supports onboarding, guidance, and informal mentorship of CHR staff as needed.
  • Identifies workflow inefficiencies and communicates improvement opportunities to leadership.
  • Organize and participate in health events, screenings, and education sessions
  • Requires adherence to infection control, safety, and confidentiality standards (HIPAA)
  • Other duties as assigned.

 

REPORTS TO: The CHR Director will report to Tribal Administrator.

 

PHYSICAL DEMANDS

  • Frequently required to stand, walk, and move throughout the facility for extended periods of time
  • Regularly required to lift, carry, push, or pull patients, medical supplies, equipment, and medications (up to 25–50 pounds, with or without assistance)
  • Ability to assist with patient transport including wheelchairs, scooters, walkers and crutches
  • Frequent bending, stooping, reaching, kneeling, and twisting during daily tasks
  • Manual dexterity required for phone use, documentation, scheduling systems, and handling medical supplies
  • Ability to remain physically active throughout the workday with minimal sedentary time
  • Visual and auditory acuity sufficient to safely monitor patients, respond to alarms, answer phones, and communicate effectively
  • Ability to respond quickly to urgent situations, including patient needs and operational issues
  • Ability to drive long distances

 

WORK ENVIRONMENT      

  • Works in a variety of settings, including office spaces, healthcare facilities, patient homes, and community locations
  • Regularly operates a vehicle for work-related duties, including patient transport and delivery of medical supplies and medications
  • Provides services in hospitals, clinics, and outpatient settings as needed
  • May work in patient homes, requiring adaptability to varying home environments
Qualifications:

QUALIFICATIONS

  • Minimum Education: Must possess an Associate’s Degree in Health or a similar field.
  • Must possess a minimum of three (3) years of supervisory work experience.
  • Must possess a minimum of two (2) years of work experience in a healthcare program.
  • Experience in a leadership role is preferred.
  • CPR/BLS certifications.
  • Ability to collect, summarize and report data coherently and logically.
  • Advanced computer skills to sufficiently input and record client and program data: prepare reports as required and requested.
  • Must demonstrate awareness, sensitivity, and appreciation of Native American culture, traditions, customs and needs.
  • Excellent interpersonal and communication skills and empathetic in client interactions.
  • High standards of ethics and confidentiality when handling sensitive information.

PREFERRED QUALIFICATIONS:

  • Preferred: Healthcare background or active Oklahoma State Licensed Practical Nurse (LPN) certification. 
  • Preferred: Grant writing experience or skills with previous administration of federal, state and/or local grants.

 

REQUIREMENTS:

  • Must be willing to submit to and pass a pre-employment and drug test. 
  • Must be willing to submit to and pass a thorough background investigation.
  • Must have a valid Oklahoma driver’s license and be insurable under the CN Motor Vehicle Policy.
