We are hiring a new Primary Care Nurse Practitioner for our Lafeyette Family Care office!
Columbia Memorial Hospital located in the upper Hudson Valley region of Upstate New York, 2 hours north of NY City and 45 minutes south of Albany, NY. It is a 192-bed facility with 42 outpatient Care Centers whose philosophy is “Patients First,” emphasizing safety and quality in patient care. The area offers the bestin/outdoor activities year-round as well as an extensive array of cultural and artistic venues.

Job Description:
Summary/Objective: We are looking for an experienced full time NP/PA to work in our Coxsackie office. The position is for an outpatient practitioner. The applicant would be part of a hospital owned primary care group practice. You will have the support of a strong administrative team. Our goal is to help you shape it to your practice style.
Setting Type (in or outpatient or both): Outpatient office
Facility Description: Primary care office in Coxsackie, New York.
Other Staff (MD’s, NP/PA’s, MA’s): 1 physician, 1 nurse practitioner, 1 physician assistant
Patients Per Day: To be determined
Hours: Negotiable.
Call: Schedule: Approximately 1/7
Type: Phone call only
Rounding: None
Reason for Recruitment: Nurse Practitioner transferring to another location
Start Date: As soon as you are able, we are flexible
