Licensed Practical Nurse (LPN) Job Description – Long-Term Care Facility
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for promoting residents' health, comfort, and well-being by delivering compassionate, high-quality nursing care while complying with all federal, state, and facility regulations.
Essential Duties and Responsibilities
- Administer prescribed medications and treatments according to physician orders.
- Assess residents' physical and emotional conditions and report changes to the RN or physician.
- Monitor and document residents' vital signs, pain levels, and overall health status.
- Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team.
- Provide wound care, catheter care, tube feeding, and other skilled nursing procedures within the LPN scope of practice.
- Supervise and delegate appropriate tasks to Certified Nursing Assistants (CNAs).
- Ensure accurate and timely documentation in residents' medical records.
- Respond promptly to resident emergencies and initiate appropriate interventions.
- Educate residents and family members regarding medications, treatments, and health conditions.
- Maintain infection prevention and control standards.
- Communicate effectively with physicians, nurses, therapists, residents, families, and other healthcare professionals.
- Promote residents' dignity, privacy, and independence.
- Participate in shift reports, care conferences, and quality improvement activities.
- Ensure compliance with all facility policies and applicable state and federal regulations.
Qualifications
- Current, unrestricted Licensed Practical Nurse (LPN) license.
- Current CPR/BLS certification.
- Previous experience in long-term care, rehabilitation, or skilled nursing is preferred.
- Knowledge of geriatric nursing practices and resident-centered care.
- Strong communication, organizational, and critical thinking skills.
- Ability to work effectively as part of a multidisciplinary healthcare team.
Physical Requirements
- Ability to stand, walk, bend, lift, and transfer residents as needed.
- Ability to lift up to 50 pounds with or without assistance.
- Ability to respond quickly to emergencies.
- Ability to perform repetitive tasks and work extended shifts when necessary.
Work Environment
The LPN works in a long-term care setting that provides 24-hour nursing services. This position may require working days, evenings, nights, weekends, holidays, and on-call shifts.
