General Purpose
To provide services to meet the social and/or emotional needs that affect the residents’ ability to achieve their highest level of function; participate in the development of resident’s comprehensive care plans; develop policies and procedures to provide social services to residents in compliance with federal, state, and local regulations.
Minimum qualifications
· Bachelors or master’s degree in behavioral sciences
· Associates degree in behavioral sciences with 5 years’ experience in a health care setting.
or
· Receive regular on-site consultation, no less often than quarterly, from an individual who has a bachelors or a master’s degree in social work or related behavioral science, and three years’ experience in a long-term care setting.
· Above average ability to read/comprehend medical terminology and records, follow written and verbal directions, and write at a level necessary to accomplish the job.
· Must be capable of maintaining regular attendance.
· Must be capable of performing the essential functions of this job, with or without reasonable accommodation.
