What you will do:
We are looking for a CARE COORDINATOR I who can help shape our vision and support our mission. Here is what the position will look like.
Education: Bachelor’s degree in human behavioral sciences field. A Combination of education and experience can be substituted for the degree with a preferred 8-years progressive experience in healthcare.
Bachelor’s degree in human behavioral sciences or healthcare related field required. A combination of education and experience can be substituted for the degree, with a minimum of 8 years of progressive experience in healthcare can be substituted in lieu of a bachelor’s degree.
Experience: Prior care coordination experience preferred.
Knowledge, Skills, and Abilities: Knowledge of care coordination. Knowledge of and ability to establish and maintain connections with members of diverse populations and provide culturally sensitive services. Ability to process high volume of work efficiently with a high level of customer service detail. Excellent written and verbal communication skills, strong organizational and time management skills, strong interpersonal skills and the ability to handle and prioritize multiple tasks and/or respond to time-sensitive requests and/or issues. Flexibility to use complex processes and procedures to facilitate quality outcomes and/or resolutions while working with other team members with varied skills and educational background.
Ability to adapt to an ever-changing work environment and operational expectations. Ability to establish and maintain working relationships with internal and external stakeholders including providers, community agencies, and state and local government. Ability to align intervention and documentation with organizational contracts, deliverables and reports. Must have general computer skills and the ability to work with Microsoft Office products and the company electronic health record. Works effectively with others towards achieving a common goal. Demonstrates support for the company’s mission, vision and values. Ability to perform health assessments and develop care plans. Knowledge of common health conditions. Motivational interviewing experience. Knowledge of utilization management. Knowledge of public assistance programs including Medicaid, Child Health Plus, and other payers, as well as managed care, supplemental insurance and/or other insurance types. Familiarity of local health agencies, community agencies, and community resources Bilingual fluent in English and Spanish preferred
Licenses/Certifications: A valid driver's license and proof of current auto insurance will be required for any position requiring driving.

Pay, Perks and Benefits at Colorado Access:
The compensation for this position is $72,464.00 - $75,300.00 annually. Colorado Access has provided a compensation range that represents its good faith estimate of what Colorado Access may pay for the position at the time of posting. Colorado Access may ultimately pay more or less than the posted compensation range. The salary offered to the selected candidate will be determined based on factors such as the qualifications of the selected candidate, departmental budget availability, internal salary equity considerations, and available market information, but not based on a candidate’s sex or any other protected status.

What you will do:
We are looking for a CARE COORDINATOR I who can help shape our vision and support our mission. Here is what the position will look like.
- Completes mandatory and needs based health assessments to identify member strengths, needs, concerns and preferences through interviewing, and utilization of standardized tools.
- Provides care coordination services and interventions by referring, educating, and coordinating with the member and their care team- members of the care team include but are not limited to healthcare providers, advocates, case management agencies, community-based organizations, state agencies, D-SNPs and other health plans.
- Educates members regarding various state plan benefits, programs, options and services.
- Monitors the ongoing provision of and need for care by assessing the delivery and quality of services and interventions provided by external providers.
- Establishes professional and effective collaboration, communication, and coordination among all responsible parties of an individual member’s interdisciplinary care team.
- As needed, attends member focused meetings (internal/external) such as interdisciplinary team (IDT), creative solutions (CS), or complex service solutions (CSS)
- Maintains knowledge of regulations, policies, and procedures regarding current public assistance programs.
- Assists members and providers in understanding the complaint, grievance and appeal process.
- as assigned by the department management team.
- Participates in training and staff development opportunities. Actively participates in team meetings and communicates progress and barriers with Coordinator Supervisor and/or Department Manager.
