Home Care Coordinator
- 1 years of Home Health Care or Case Manager experience
- Knowledge of Medicaid waivers and the state EVV system preferred
- High attention to detail
- Exited self-starter, with the ability to lead and problem solve
- Excellent written and verbal skills and proficiency with computers
Position responsibilities:
- Communication daily with clients, caregivers, families, and 3rd parties
- Attend designated training and ongoing education opportunities
- Providing assistance with the onboarding process to potential clients with Care Coordinators/Case Managers.
