Schedule: This position is part of a strong nurse management team, utilizing a very high ratio of nurse managers to residents to oversee a small skilled nursing unit within a continuing care retirement community. The Admissions Coordinator makes assessments for, plans, and advocates for the residents of the Community so they can reach their highest practical level of physical, mental, and psychosocial well-being. The Admissions Coordinator will be responsible for pre-admission assessment of referrals as well as discharge planning for all the residents of the Community. Has the ability to communicate effectively and advocate for resident centered/directed care to team members, families, physicians, allied health professionals, and the at-large community.

Role

Responsibilities
- Follows and upholds the Community’s philosophy and goal to maintain, improve, and/or enhance each resident’s quality of care and quality of life
- Performs pre-assessment of residents as necessary.
- Attends and participates in facility meetings and in-service training programs as assigned.
- Participates in the nurse on-call rotation
- Promotes positive public relations with residents, family members, guests, and outside referral sources.
- Collects and assesses data relevant to residents’ psychosocial needs, risk factors for psychosocial deterioration, and responses to interventions.
- Completes assigned portions of the MDS.
- Maintains referral tracker.
- Coordinates admissions and discharge planning
- Reports monthly discharges to the Ombudsman.
- Coordinates and directs the care plan meetings.
- Facilitates the advanced directive decision-making process
- Coordinates the grievance complaint process
- Coordinates and assists with resourcing and scheduling of allied health services, i.e., dentists, Podiatrists, Medical specialists, etc.
