What are the responsibilities and job description for the QMHP-Mental Health Deputy Care Coordinator position at COLLIN COUNTY MENTAL HEALTH RETARDATION CENTER?

The Mental Health Deputy Care Coordinator reports directly to the Program Administrator of Crisis Outreach Services and works closely with the Collin County Sheriff’s Office. This position uses initiative and passionate commitment to provide care coordination to individuals in the Mental Health Deputy Co-Responder Program to ensure their success in recovery. Care Coordination responsibilities include ensuring individuals receive timely access to quality care, working with the entire LifePath Systems team and other community organizations to identify needs and reduce barriers to care. Must be effective at troubleshooting and organizing support, interventions, and services to achieve outcomes. This position will ensure a seamless transition for individuals receiving services across a full spectrum of health and social services, provide referrals internally and externally as needed to meet the needs of individuals, and will ensure that all gaps in care are reconciled within a trauma-informed, person-centered model of care.

 

Key responsibilities: Ensuring individuals receive timely access to appropriate services; submit monthly reporting following contract requirements; time management and organization skills, active collaboration with community partners, and strong teamwork abilities; successfully complete all required training for both LifePath Systems and Collin County Sheriff’s office upon hire and annually; other duties as assigned to support program needs and partnership collaboration.

 

Specific Targets: Ensure that 80% of individuals in the Mental Health Deputy Co-Responder Program are connected to a case manager within 14 days of a new intake or case assignment, are connected to a prescriber within 30 days of new intake or case assignment, are connected to substance use services (if applicable) within 14 days of identified need, and remain fully engaged based on the individual’s need. Responsible for assessing individual served needs and providing referrals and support. 90% of service notes are signed within 2 business days of service. 80% of full chart reviews receive a score of 80 or higher. All required trainings are completed prior to the due date. Attends monthly team meetings.

 

In the event of unforeseen circumstances that require the use of Paid Time off, employees are expected to notify their supervisor within 4 hours of their regularly scheduled work time.

Qualifications:

Education, Training, & Experience (including licensure & certification)

  • Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
  • Behavioral Health and specifically Crisis experience preferred.
  • Knowledge of Trauma-Informed Care Practices required.
  • Previous work experience, partnership, or collaboration with law enforcement highly desired.
  • Must have reliable transportation and a valid driver’s license.
  • Must undergo and pass a background check with Collin County Sheriff’s Office.

  • Skilled in crisis intervention techniques and communication
  • Ability to respond to individuals with patience, empathy, and understanding
  • Ability to work within a team, both providing and receiving direction in a collaborative approach and taking initiative
  • Knowledge of the social services system and the ability to connect individuals with appropriate services
  • Ability to effectively communicate regarding behavioral health needs and services
  • Ability to work under general supervision with moderate latitude for the use of initiative and independent judgment
  • Knowledge of the social services system and the ability to guide staff in connecting individuals with appropriate services, including resources for those with housing instability
  • Knowledge of UM Guidelines, Texas Administrative Code, and Info Item V Standards
  • Organized, detail oriented, computer proficient, and able to write clearly and professionally
  • Possess the ability to think clearly and logically, to deal with potentially dangerous and/or emotional situations and utilize de-escalation techniques.
  • Ability to assess individual’s needs, to coordinate services, to communicate effectively, and to train others.
